well, dvd subs are already in vobsub format, for extracting them from the video_ts, i'd use avidemux, i think ffmpegx might be able to do it too, but not very well.
I think ffmpegX can (badly) try and multiplex a video, audio, and subtitle file together, but I don't think it can extract a VOB subtitle. Also those utilities I have seen that can do this, don't generate the idx file, merely the raw subtitles. I use a Windows utility called Rejig 0.5 which can demux VOB subtitles and remux back in to a new VIDEO_TS but it does not produce an idx file (and of course is not for the Mac).
